急性淋巴细胞白血病复发患儿应用不同化疗方案的临床疗效对比研究

目的 :探究急性淋巴细胞白血病复发患儿应用不同化疗方案的临床疗效差异。方法 :选取我院2015年12月—2017年12月收治的急性淋巴细胞白血病（acute lymphoblastic leukemia, ALL）复发患儿62例,根据化疗方案的不同分为观察组（35例）和对照组（27例）。对照组采用包含环磷酰胺（cyclophosphamide）、泼尼松（prednisone）、多柔比星（doxorubicin）、左旋门冬酰胺酶（l-asparaginase）、长春地辛（vincristine）的CODLP方案;观察组采用包含地塞米松（dexamethasone）、多柔比星（doxorubicin）、培门冬酶（pegaspargase）、长春地辛（vincristine）的VDLD方案。比较两组患儿诱导缓解率、诱导期死亡率和并发症发生率以及1年内生存情况。结果 :两组诱导缓解率、诱导期死亡率比较,差异无统计学意义（P> 0.05）;观察组并发症发生率低于对照组,差异有统计学意义（P <0.05）;两组随访期（12个月）内,两组总生存率比较差异无统计学意义（P> 0.05）;观察组无病生存率高于对照组,差异有统计学意义（P <0.05）。结论 :与CODLP方案比较,采用VDLD化疗方案治疗ALL复发患儿在诱导缓解疗效相当的情况下,可更有效降低化疗相关并发症发生率,提高患儿生存状况,值得临床推广使用。
